First Day Packet and Required Forms

Welcome to the 2020-2021 school year! Each year, the District provides all families with important annual notifications that require your signature.

This year, because all students are beginning in a remote-learning setting and will not be present on campus, hard copy forms will not be distributed. Instead, all forms and required reading materials are available on the district website at

We ask that you please review the required reading on our website, and then return the following three forms to your school site via email (preferred) or in person:

  1. Student Health Information Card
  2. Signature Verification Form/Emergency Information
  3. Technology Acceptable Use Parent Acknowledgement Form

Please return all three required forms by the end of the day on Friday, August 28.
